Please bring any insurance information with you to the consultation so that we can verify and review your insurance coverage. Please be aware that most services are covered by dental, not medical, insurance. Facial cosmetic services are not covered by insurance. Your estimated payment is due at the time service is rendered. We usually give a receipt the day of surgery, and if that is not done, please ask and we will gladly provide one. If there is any remaining balance after your insurance has paid, it is your responsibility. Most insurance companies will respond within four to six weeks. We will give you a call, or send you a statement if a payment is due, Your prompt remittance is appreciated. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility.
